## It is not the aligner that moves the tooth, it is the bone

An aligner looks like a thin impression of your row of teeth, but it is deliberately shaped a little differently. It sits on all the teeth at once and presses where the next step is meant to go. This pressure is light and constant, and that is exactly what matters.

What happens next takes place not in the tooth but around it. The tooth is not fixed rigidly in the jaw; it hangs in a narrow gap of fibers between root and bone. On the side it is meant to move toward, the body breaks bone down. On the opposite side it builds new bone up. So the tooth does not travel through the bone, the bone is rebuilt around it.

That is why it goes slowly, and why patience here is not a drawback but a condition. Too much force speeds nothing up, it does harm. Each aligner moves the teeth only a small step, then the next one follows.

### What the attachments do

Some movements are beyond a smooth aligner on its own, rotating a tooth for example, or drawing it down out of the row. For those we bond small tooth-colored bumps of composite onto individual teeth, known as attachments. The aligner grips them like a handle and can transmit forces that would otherwise slip off.

The attachments stay on the tooth for the whole treatment and are removed without residue at the end. They are tooth-colored; from a meter away they are barely noticeable.

## Three case sizes, the same technique

Aligners are not a single product but a family. What sets them apart is the region of the jaw in which teeth are moved, and how many aligner steps are available for it. The further back the movement reaches, the more extensive the plan.

Which size is an option for you follows from the findings, not from the wish. We clarify that before the first scan.

The difference is not a question of quality. All three work with the same material, the same planning process and the same check-ups. What grows is the scope: more teeth in motion, more steps, more time.

## Why we do not treat the upper and lower jaw separately

Technically it would be possible to treat only one jaw, and it is offered. Usually the request then goes: only the lower front teeth, they are crooked, the rest is fine.

We do not do this, and the reason lies in the bite. Upper and lower jaw work against each other, every tooth has an opposing tooth. If you change the position below, you also change where contact arises above. A row that looks straight on its own can fit worse than before once both jaws work together.

That is why we plan both jaws together and judge the result by the way the teeth meet, not by a photo from the front. This does not make the treatment more complicated, it makes it predictable.

## From the first look to the last aligner

1. **Consultation and findings** — We look at your teeth, gums and bite and take the necessary X-rays. At the end there is a statement on whether aligners are the right means in your case, and which case size would apply. Sometimes the answer is that something else has to be treated first. — Cavities and gum inflammation are treated first.
2. **Digital scan** — A small camera passes over both jaws and the way they meet. From this a three-dimensional model is created. No impression material, no full tray.
3. **The plan on screen** — On the model the movement is designed step by step: which tooth goes where and when, how many aligners it takes, where attachments are needed. You see the planned course before anything is made. — What the software proposes is checked and adjusted.
4. **Attachments and the first aligner** — The attachments are bonded, the first aligner is fitted and its seat is checked. You take several aligners home with you and know when to change them. — For the first few days the teeth feel sensitive to pressure.
5. **Wearing and changing** — You wear the aligner day and night and take it out for eating and brushing. After the planned interval the next one follows.
6. **Check-ups** — At intervals we check whether the teeth are following the plan. If they are not, we adjust course, early rather than at the end.
7. **Refinement, if needed** — If the result deviates from the plan, we scan again and a further set of aligners is made, known in the profession as a refinement. That is not a mistake, it is planned for. — Refinements are part of the agreed scope of treatment.
8. **Completion and retention** — The attachments come off, the teeth are polished. Securing the result begins directly afterward. Why this is not an afterthought is explained further [below](#danach).

## What the treatment asks of you

Aligners have one advantage that is at the same time their greatest weakness: you can take them out. The result therefore depends on you more than it does with fixed braces.

- **About 20 to 22 hours a day.** This is not a recommendation, it is the condition the plan is calculated on. Anyone who stays clearly below it not only pushes back the end, but also throws the sequence out of order.
- **Take it out to eat and for anything other than water.** Sugary or acidic drinks sit against the tooth under the aligner for minutes on end, with no saliva to dilute them. Coffee and tea also stain.
- **Brush before putting it back in.** Whatever is left on the tooth gets sealed in by the aligner. Skipping this risks cavities in exactly the places where no one expects them later.
- **Clean the aligner itself.** Cold or lukewarm, with a soft brush, without toothpaste, which is abrasive. Hot water warps the material.
- **Speech settles in.** For the first few days S sounds are different. This usually passes within a week.
- **A feeling of pressure after every change.** For a day or two the teeth feel as if they have been working. That is exactly what has happened.

If you suspect that 22 hours a day is not realistic in your everyday life, say so before treatment begins and not after the third change of aligner. There are cases in which a fixed appliance is the more honest route.

## Where aligners are not the right means

Transparent aligners can do a great deal, but not everything, and the limit does not run where advertising draws it. We clarify these points before the planning, not halfway through.

- **The teeth are not healthy.** Cavities, untreated [periodontitis](https://f80.berlin/en/treatments/periodontitis/) or inflammation at the root are treated first. Moving teeth in inflamed tissue makes the situation worse.
- **The case reaches beyond the range of movement.** If the molars further back have to move as well, or the position of the jaw itself has to be changed, that is comprehensive orthodontic treatment. In that case we refer you on rather than working at the edge of what is feasible.
- **Growth is involved.** Different considerations apply to children and teenagers who are still growing. For teenagers there is a separate route, see [Invisalign Teen](https://f80.berlin/en/treatments/invisalign-teen/).
- **Teeth are missing or heavily restored.** Crowns and bridges hold attachments less well than natural enamel. Gaps change the mechanics of the dental arch. Neither rules out aligners, but both belong in the conversation beforehand.
- **The expectation does not match the findings.** Straightening changes the position of the teeth. It does not change their shape, color or length. - **You grind heavily.** Clenching at night works against any plan. That does not mean aligners are ruled out, but the cause belongs on the table, see [night guard](https://f80.berlin/en/treatments/night-guard-in-berlin/).

## The aligner can do one more thing

Anyone wearing an aligner that fits their own teeth exactly already has in their mouth what teeth whitening requires. There is a gel developed for precisely this carrier, formulated to run thinner than usual whitening gels.

It can be used before, during or after the correction, and also while the attachments are still bonded on. In most cases we discuss it at the end of the treatment: once crowded teeth have moved apart, surfaces are exposed that no one has seen before.

## The last aligner is not the end

When the teeth are where they are meant to be, the treatment has reached a point that many take for the goal. Professionally it is the transition into a second phase.

The bone that carries the teeth is still being remodeled at this point. On the side a tooth has moved toward, bone was broken down; on the other side the new bone is not finished yet. So the tooth stands in the right place, but not yet firmly. Without retention it moves back in this phase, and it does so reliably.

## What determines the cost

There is no fixed price for “an aligner treatment”. The German scale of fees for dentists (Gebührenordnung für Zahnärzte) requires the fee to be set according to the effort and difficulty of the individual case. What your treatment costs is therefore settled only after the findings and the planning, but then in writing and in full, before anything begins.

### Factors that affect the cost

- **Extent of the case** — How many teeth are moved and over what range. A short plan with few steps is a different service from one covering both jaws back to the molars.
- **Number of aligner steps** — It follows from the plan, not from a package. Every step is a manufactured piece.
- **Pre-treatment** — Teeth cleaning, cavities, gum treatment or the removal of old restorations are charged separately and are listed in the plan in advance.
- **Chairside work** — The scan, the planning, bonding the attachments, the check-ups and the removal at the end are services in their own right.
- **Retention** — The bonded wire and the removable set of retainer trays are part of completing the treatment and are in the cost estimate from the start.

### What the plan already includes

Two things that are rarely asked about and that make the difference later: **refinements and replacement aligners are part of the agreed scope.** If the result has to be adjusted at the end, the additional set of aligners is planned for. And if you lose an aligner along the way, it is replaced without a new invoice arising from it.

We write this not because it would be generous but because it makes the treatment predictable. You should not have to weigh the cost every time you have a question.

### What statutory health insurance covers

For adults, statutory health insurance covers orthodontic treatment only in narrowly defined exceptional cases, for instance severe jaw misalignments that are treated together with surgery. A correction for aesthetic reasons is not among them. Private policies and supplemental plans provide benefits differently; submit our plan for review before treatment begins.

## Frequently asked questions about Invisalign

### Can the aligners be seen?

Close up and on careful inspection, yes. They are transparent and thin, but not invisible, and the attachments sit visibly on the tooth, tooth-colored though they are. In conversation most people notice neither.

### Does it hurt?

After every change you feel pressure for one to two days, above all when biting down. That is the force that sets the remodeling in motion, and it eases off. Lasting pain is not part of it; get in touch if that happens.

### How long does the treatment take?

That depends on the number of steps and on how consistently you wear them. The plan gives you a figure before you decide. Anyone who regularly leaves the aligner out not only extends the treatment but may have to have it planned again.

### Can I smoke or drink coffee with them in?

The aligner should be out for that. Pigments are deposited in the material, and under the aligner they sit directly against the tooth without saliva.

### What happens if I lose an aligner?

Get in touch and we will work out whether you go back to the previous one or start the next one earlier. Replacement is included in the scope of treatment. What does not make sense is carrying on for days without an aligner.

### Why do I need attachments when someone I know did not?

Because the movements needed differ. Rotating teeth, tipping them or changing their height calls for a point of purchase that a smooth aligner does not have. Simply aligning minor crowding sometimes manages without.

### Is this the same as the aligners you order online?

No. Mail-order providers leave out the examination before the start, the X-ray and the monitoring during the movement. That is precisely where it is decided whether a movement harms the supporting structures of the tooth. We do not treat cases that were planned without clinical findings.

### I had braces as a teenager, why are my teeth crooked again?

This is common and not a personal failing: without lasting retention, teeth move back. For small relapses a short plan is often enough. What matters more than the correction is how the result is secured afterward, see [Retainer](https://f80.berlin/behandlungen/retainer/).

### Do I really have to wear something afterward?

Yes. This is not an optional extra, it is the part without which the result does not hold. We settle it before the start and put it in the cost estimate.
